The Digital Rectal Exam is the standard prostate exam employed by doctors around the world. It’s also commonly the butt of jokes the world over, owing to how uncomfortable the subject matter tends to be among men.

And let’s not beat around the bush, the first time you get a digital rectal exam, you’re going to feel awkward, and probably a little embarrassed. This is understandable, but really, there’s nothing at all to feel ashamed or embarrassed about. Within moments the whole thing will be over, at which point you can either forget about it or come to the realization that, hey, that wasn’t all that bad after all.

But just to allay your fears a little more, let’s take a slightly deeper look at this prostate exam and what exactly you can expect from it, so that when that moment comes for you to have it performed, you’ll be fully prepared.

 

The Prosate Exam Drill

The first step will be to strip off your clothes and strap yourself in one of those charming hospital gowns with the strips down the back. Because the prostate is located near the rectum, you’ll need to bend over so the doctor can gain easier access to it. This can be accomplished either by bending over while standing up and resting your arms on the table, or by lying down on your side and pulling your knees up to your chest.

The doctor will probably place one hand on your abdomen to help keep you steady while he performs the exam. He’ll then separate your buttocks and insert his gloved, lubricated hand into your rectum (unfortunately, it doesn’t sound great no matter how you say it, but there it is). Because you’ll likely tense up, the doctor will pause briefly at this point to allow you to relax before moving his hand further in, towards your prostate.

Once the doctor has reached his destination, he’ll begin to perform the prostate exam. He’ll have a checklist of things he’ll be looking for, starting with the size, which would tell him whether or not you have an enlarged prostate. He’ll also be feeling for lumps, which, when combined with an enlargement, could indicate the presence of prostatitis. Lastly, he’ll be trying to determine the relative firmness of the prostate gland. An overly firm prostate could mean cancer is present in the prostate.

During this process you may begin to feel faint or nauseous, and may also feel the need to urinate as your prostate is being touched. These are all natural reactions, and nothing to feel embarrassed about. Within a minute or two, the whole ordeal will be over, and the doctor should have a very good idea concerning the general state of your prostate.

 

Some additional tips you can put into practice while getting a prostate exam:

 You may feel like you want to hold your breath, but refrain from doing so. Continue to breathe in a normal fashion, or at a slightly reduced pace to control your stress.

 Try to visualize being in another place, or focus your mind on something else. Think about something that makes you happy, like your kids, a favourite hobby, or a place where you feel the most in peace.

  If you’re feeling any pain, or getting lightheaded at all, don’t be afraid to let the doctor know. The presence of pain in particular could signal an existing condition.

Fine. I’m intrigued. What exactly is the prostate?

It’s a walnut-sized gland nestled deep inside the male pelvis, and it helps jack semen up with important nutrients, Mark Hong, M.D., a urologist in Phoenix, Ariz., tells SELF. The prostate is made up of tissue known as stroma, secretory glands that produce semen ingredients, and muscle fibers. It’s located right in front of the bladder—the urethra actually runs through the prostate, which is why prostate issues can negatively affect men’s urinary function.

The anus and its surrounding area are packed with nerve endings, says Hong. That’s precisely why anal play can feel so amazing whether you’re having sex with a man or woman. Hong also notes that the prostate has plenty of blood vessels that swell with increased blood flow as part of the normal sexual response cycle. The extra blood helps make the area more sensitive, which can boost pleasure.

Unfortunately, in addition to nerve endings, the anus is also surrounded by stigma. «[Anal play] is often considered to be dirty, and you might hear jokes about [the anus] being ‘exit only,'» Lexx Brown-James, Ph.D., L.M.F.T., tells SELF. «But that kind of thinking limits the pleasure you can enjoy.» The human body is amazing in that parts like the vagina and anus have natural functions but can also feel really good. Trying out different sex-related acts doesn’t make you dirty, just normal.

So, how do you both stay safe while experimenting with prostate stimulation?

When it comes to sex, there are two kinds of safety: physical and psychological. That psychological safety is even more important than usual when doing something «taboo» like exploring a man’s anal area, says Hong. «Safety starts with permission,» he explains. Brown-James agrees, saying prostate stimulation «is not something you surprise [men] with—it takes some preparing.» In most cases, that means bringing it up verbally before getting down to business. A simple «You know, there’s something I’ve been really excited about trying…» can help start the conversation, says Brown-James.

Next up is physical safety, aka avoiding anal trauma. «The anal mucosa [lining of the anus] and the anus itself are very sensitive,» says Hong. Although the anus is built to stretch to a certain extent, it doesn’t self-lubricate, meaning without proper precautions anal stimulation can cause pain, tearing, or even infection.

The first step in preventing that is washing your hands with good old soap and water, per these handy instructions from the Centers for Disease Control and Prevention. After that, you can slip on a latex glove, if you like. It can protect both of you by making sure you don’t transfer any anal bacteria to his penis or your vagina post-prostate action. All set on the safety front. What now?

Encourage your partner to take deep breaths, because a relaxed anal sphincter is a happy anal sphincter (and one that’s way easier to penetrate). Also, make sure he’s turned on as hell. Not only will that make it better emotionally, it’ll increase blood flow to his genital area, likely making it feel better for him physically as well. And just like with female G-spot stimulation, a prostate massage can make a guy feel like he needs to pee, so it can be a good idea for him to empty his bladder beforehand.

You can first access the prostate via the perineum, a spot right in between the testicles and the anus. «Bring your pointer, middle, and ring fingers together, then push up into that patch of skin,» says Brown-James. «Stimulating this area with gentle but firm strokes also stimulates the prostate.» It’s basically a playground down there—Brown-James recommends techniques like rubbing clockwise for 8-10 strokes, then going counterclockwise, stroking towards his scrotum, or touching his penis while you massage his perineum. Basically just test things out and ask what feels good!

If you’d like to move on from the perineum and actually enter the anus, lube is essential. Keep in mind that oil-based lubricant eats away at condoms, and silicone-based ones can damage silicone sex toys. But don’t use any numbing agents, either with or without lube—pain is there to tell you to stop, says Brown-James. Once your finger (you should probably use your pinky unless he wants something bigger) is adequately lubed up, you can try rubbing his anus and the surrounding area, then sliding it in.

As someone who’s performed thousands of prostate exams, Hong emphasizes the importance of going at a pace that’s comfortable for your partner. Once you’ve got a finger in and your partner’s go-ahead, start moving it upwards in a «come here» motion towards the front of his body, says Brown-James. Because the prostate is a bit further in and higher than the anus, that upward motion is a good way to make contact.

And if you’re using a sex toy, only opt for one that is specifically made for anal play, meaning it has a flared base or other trustworthy retrieval method. «Every physician who’s ever worked in the ER has a story of a household anal sex toy gone awry,» says Hong. «If you can imagine it, it’s probably been up the rectum.»

If this is all feeling good to your partner, he may orgasm from the stimulation. Don’t be nervous if he climaxes but doesn’t actually ejaculate—according to Brown-James, that can happen during prostate play. «It feels the same, but the semen goes back into the bladder instead of leaving the urethra,» she explains. It’s not a health risk, according to Hong, so if it happens, chalk it up as yet another fascinating part of having sex.
